The rehabilitation and skilled nursing areas of Swift Creek Health Center provide accommodations to address needs that arise from the unforeseen circumstances of growing older. Whether the condition is the result of a fall, a joint replacement, a heart attack or a stroke, it is the mission of our rehabilitation center to restore residents to their highest level of functioning. For those who need longer term solutions, Swift Creek’s skilled nursing services provide ongoing attention from RNs, LPNs and CNAs, all under the supervision of a MD who serves as the medical director.
