Inspections, Administrators and Resident Rights Under Tennessee's Rule
Nine licenses in the city. On the Health Facilities Commission's October 2026 bed report, nine assisted care living facilities have a Murfreesboro, TN, address. Seven offer assisted living. The Rutherford Memory Care is devoted to memory care. The Villages of Murfreesboro, licensed as The Village of Murfreesboro, pairs independent living with a licensed memory care neighborhood.
A certified administrator. Each community needs a state-certified administrator. The one exception is someone who already holds a Tennessee nursing home administrator license. The certificate renews every two years, and the administrator shows at least 24 classroom hours of training to renew, on topics such as state rules, nutrition and money management.
Visits without warning. State rules call for an unannounced inspection of each community within 15 months of its last one. When inspectors find a broken rule, the community gets a written statement of deficiencies. It then has 10 days to send back a plan of correction, its written plan for fixing each problem and keeping it from coming back.
When nursing care is needed all the time. A Tennessee assisted care community can't admit someone who needs continuous nursing care, and it can't keep a resident who comes to need it. In Murfreesboro, TN, Adams Place runs a 90-bed nursing home on its campus, and Stones River Manor holds a 30-bed nursing home license.
A say in the agreement. A resident has the right to help write the terms of the admission agreement. Those terms cover choices about doctors, hospital and nursing home care, medicines, emergency plans and funeral plans. At move-in, each resident gets a copy of their rights to read and sign.
Speaking up safely. Residents may complain and suggest changes without pressure or punishment. The community has to explain how to file a complaint in confidence. It also can't punish anyone who makes a good-faith complaint to the state or to Adult Protective Services.
Staff checks and public records. The state's Abuse Registry is open for anyone to search online, and a community can't hire anyone on it. Each month, the commission also posts a list of the actions it has taken against the places it licenses.
Questions to ask on a tour:
- At The Rutherford Assisted Living, what does the free assessment look at, and how often is it redone?
- At Harmony at Victory Station, how much does the care part add to the rent?
- At Adams Place, how does a move to the nursing home on campus work?
- When was the last state inspection, and can I see any plan of correction?
- Can I take the admission agreement home to read before signing?
TennCare's $2,982 Income Limit and Other Ways to Pay in Murfreesboro, TN
Medicare.gov says Medicare and Medicare Supplement Insurance (Medigap) don't pay for long-term care. Most of it is non-medical help with daily tasks, which Medicare.gov says can be given at home, in the community or in an assisted living facility. So private pay comes first for you or your family member. Public programs help people who qualify.
- Private pay. It can draw on a pension, savings or the sale of a house.
- Long-term care insurance. The policy's terms set what it pays. Tennessee's Department of Commerce and Insurance explains Partnership policies: one generally protects assets equal to what it paid out, but Medicaid's other rules still apply.
- TennCare CHOICES. TennCare is Tennessee's Medicaid program. Its CHOICES program lists assisted care living facility services for Group 2 and Group 3 members, who pay their own room and board. For 2026, monthly income can't be more than $2,982, though someone above that may be able to set up a Qualifying Income Trust. Countable assets can't be more than $2,000, not counting the home, and TennCare looks back five years for anything given away or sold for less than it's worth. The Greater Nashville Regional Council (GNRC) helps people who aren't on TennCare yet apply. TennCare's PACE program, a different route, lists Hamilton County residents only.
- VA Aid and Attendance. It's a monthly amount added to a VA pension, for veterans and survivors who get the pension and meet a need such as help with bathing, feeding or dressing. For a veteran with no dependents, the maximum annual pension rate with Aid and Attendance is $29,093, at rates in effect since Dec. 1, 2025. Through Nov. 30, 2026, the net worth limit is $163,699.
- OPTIONS for Community Living. This state-funded program pays for help at home as a last resort. There's no income limit, but people above a certain income pay part of the cost on a sliding scale. GNRC says every one of its 13 counties has a waiting list.
Who to Ask in Rutherford County
- GNRC's Information and Assistance helpline. GNRC is the Area Agency on Aging and Disability for 13 Middle Tennessee counties, Rutherford among them. Its counselors help sort out what's needed and whether someone may qualify for a program. The call is free, and the line is open weekdays from 8 a.m. to 4:30 p.m.
- The district long-term care ombudsman. For problems with care, money, rights, admission or discharge that a community hasn't settled, Rutherford County families can contact the Greater Nashville district office at the Mid-Cumberland Human Resource Agency. It works with residents, families and providers to resolve them, keeps matters confidential and doesn't regulate the communities.
- Complaints to the state. The Health Facilities Commission gives each complaint a priority code based on how serious it is. Then a regional office looks into it. The name of the person who complains stays confidential.
- Meals for people 60 and older. The Mid-Cumberland Human Resource Agency runs senior dining sites and home-delivered Meals on Wheels in Rutherford County, for people who are homebound or meet state guidelines.
- Hospitals. Ascension Saint Thomas Rutherford, on Medical Center Parkway, has an emergency room open 24 hours. For veterans, the Alvin C. York VA Medical Center on Lebanon Pike has an urgent care clinic. It takes walk-ins every day from 8 a.m. to 8 p.m., but it can't accept patients who arrive by ambulance.
Frequently Asked Questions
Posted starting prices in Murfreesboro, TN, run from $2,650 a month at The Rutherford Assisted Living to $5,060 at Harmony at Victory Station. Park View Meadows lists $4,320 (as published, checked Oct. 20, 2026). Harmony's price has two parts, rent and care, and Park View Meadows calls its price all-inclusive. CareScout's 2025 median for the Nashville area, a private one-bedroom paid privately, was $6,460.
No. Medicare.gov says Medicare doesn't pay for long-term care, and it names assisted living as one place people get that kind of help. Medicare costs can still shape a family's budget. GNRC's free Medicare counseling, part of Tennessee's State Health Insurance Assistance Program, can check whether a Medicare Savings Program would help with Part B premiums.
TennCare CHOICES is the program to ask about. Its 2026 money limits are $2,982 a month in income, though a Qualifying Income Trust may help someone above that, and $2,000 in countable assets, not counting the home. GNRC helps with the application. CHOICES members pay their own room and board. For care at home instead, OPTIONS for Community Living can help, with a cost share above a certain income and a waiting list in every GNRC county. Mid-Cumberland runs Meals on Wheels.
At the medians, yes. CareScout's 2025 Nashville area figures were $6,460 a month for assisted living and $9,885 for a shared nursing home room. In Murfreesboro, TN, Adams Place has both on one campus, so a family can ask one community for both prices.
Not when the need is continuous. Under Tennessee's rule, an assisted care community can't admit or keep anyone who needs continuous nursing care. The rule lists a few other limits too, such as advanced pressure sores, also called bedsores.
The community is the place to start. It has to explain how to file a complaint confidentially. If that doesn't settle it, the Greater Nashville district ombudsman at the Mid-Cumberland Human Resource Agency can help. You or your family member can also file with the Health Facilities Commission. It keeps the name of the person who complains confidential. The community can't punish anyone for a complaint made in good faith.
