It is our great pleasure to announce the completion of a full building renovation! This floor to ceiling (and roof) renovation has completely transformed our entire building; all patient rooms, beautiful enhancements to the dining room, lobby, and all common spaces. This expansion increased our private rooms from 8 to 33 and we now have 15 one-bedroom assisted living apartments. Our highly skilled nursing and incomparable rehabilitation services are known throughout the long-term care industry. For those that require a little extra care after a hospital stay, our skilled nursing program provides a safe transition from the hospital to home. For those requiring some level of assistance with activities of daily living, Oak Creek's Assisted Living community provides the help you need 24 hours a day to help you lead an active and independent lifestyle. One-bedroom apartments feature a kitchenette with hardwood floor and wood cabinetry, living room, bedroom and private, full bathroom Our highly trained staff are capable of handling more acute patient needs. Long term care residents are given loving care and our team is focused on maintaining activities to keep their mind and body engaged. We currently have collaborative agreements for hospice care with Hospice of Dayton, VITAS Healthcare, and Hospice of Miami Valley to ensure your loved one has a full team of experts and emotional family support available.
