Heritage Health Care Center, located in Oakwood Village, OH offers memory care & skilled rehabilitation.

Grande Pavilion has the services and atmosphere you need from a skilled nursing and rehabilitation facility. We are located 30 minutes from Cleveland, Ohio, and provide a home-like environment to support our residents’ daily activities. Our rooms are furnished with the comforts of home, and residents are welcome to decorate their spaces with personal items. Grande Pavilion offers 80 beds, and all residents have the option of selecting a private room. Grande Pavilion also offers a specialized memory care facility. Cobblestone memory care has 50 beds in a secure environment that provides programs and support for those with Alzheimer’s and dementia. In our activities room, we host a variety of engaging events, including art classes, religious services,exercise classes, bingo and movie nights. We also arrange outings for shopping and community events. Residents are served three chef-prepared meals daily, and snacks are available 24-hours a day.
