Providing medical & personal care while promoting safety, dignity, comfort & person-centered choices in neighborhood-style settings There are 10 unique general long-term care neighborhoods (small groupings of resident rooms) from which to choose your new home. Our caregivers, including chaplains, provide care for the human spirit as well as physical health. Following our person-centered model of care, residents make as many of their own choices and decisions as possible allowing us to honor their personal preferences. Residents, caregivers, families and volunteers grow strong ties through ongoing relationships. What is short-term transitional care? Short-term transitional care is a recuperative place to heal between hospital and home. Our transitional care program has been around since 2005 to provide specialized rehabilitative care to individuals following a stay in a hospital. Working alongside you and your loved ones, our highly trained TCU team is experienced in creating a care plan which, from the very first day you arrive, helps prepare you for discharge back to home. Patients who come to the TCU must meet the admission criteria established. Once their condition has improved, they will either be discharged back to their homes or more appropriate placement. They will also have the option to stay long-term on our campus. Comfort and healing during the stay with us is our number-one goal.
