St. Luke's Transitional Care Center is specially designed to bridge the distance between a hospital setting and the place you really want to be "home." St. Luke's Transitional Care Center is a recuperative and supportive place to heal following an injury, surgery or serious illness. This facility is unique to our community. It's not a nursing home - it's a true transitional care center designed for short-term rehabilitation. Building on our expertise with St. Luke's Living Centers, our professional approach to healing includes daily, individualized therapy sessions to help patients regain the strength, stability and confidence they need to function at their best. A dedicated team of providers, nurses, social workers, physical, occupational and speech therapists, and support staff help patients manage their health, so they can focus on getting better. With UnityPoint Clinic nurse practitioners on-site five days a week, we respond quickly to a patient's changing needs, minimize visits to the hospital, coordinate care and communicate frequently with our patient's doctors. Our average length of stay is 14 days.
