The Lincoln Glen Church congregation came up with the idea for this non-profit community in 1965, and it was built in 1969 on six garden acres. It sits at 2671 Plummer Avenue in the Willow Glen neighborhood of San Jose, CA. It's a certified California Continuing Care Retirement Community with a skilled nursing license, offering four levels of care: independent living, assisted living, memory care and skilled nursing. Residences are month-to-month, and there's no entrance buy-in.

Independent living has three residence types:

• Cottage: 640 square feet, with a private patio
• One-bedroom apartment: 440 square feet
• Studio: 350 square feet

Apartment and studio residents get three meals a day in the dining room, and cottage residents get one. Independent living also has twice-monthly housekeeping, licensed nurse consultations and 24/7 emergency call response. Pets are welcome, subject to community requirements.

Assisted Living and Memory Care at Lincoln Glen Manor

Assisted living has 30 studio units and a one-bedroom suite, with medication management, weekly chapel services, and staff-led activities, music and outings. Memory care is in Sunshine Alley, a secured neighborhood on the ground floor with an outdoor garden for walking and sitting. Its care station is staffed 24/7, and licensed nurses are there around the clock for your family member.

Getting Around from Lincoln Glen Manor

Wheelchair-accessible buses and vans give scheduled rides to personal and medical appointments within a 12-mile radius. They run Monday through Friday from 9:30 AM to 3:30 PM, except holidays. When family comes for a meal, guest meals are $5 each.
