Maple Grove Nursing & Rehab has been caring for residents and members of the Lebanon community since 1990. Throughout the years, the center has continued to meet the changing needs and continuum of care required by today’s health care needs. In addition to traditional long-term care services, Maple Grove Nursing & Rehab offers premier short-term rehabilitation services and skilled nursing. Our 60-bed center is nestled in a beautiful community and is conveniently located to community resources, amenities, and other health care providers. Our unique model of care utilizes interdisciplinary teams of physicians, nurse practitioners, licensed nurses and therapists. We work closely with residents and families to develop personalized care plans based on their needs, preferences and life experiences. Our goal is to educate and empower both the caregiver and resident to take an active role in their plan of care both during and after their stay. We believe our small size and family-based approach create a warm, nurturing environment essential to healing. Residents are encouraged to decorate their own rooms, and families and friends visit when they want. Activities are structured around individual needs and include innovative programs designed to stimulate and engage each resident’s personal interests. Our dieticians make every effort to accommodate individual preferences.
