Southwestern Vermont Healthcare has a full time Nurse Practioner on site at CLR who assists with any medical needs that arise during the transition and during your stay. Whether you’re facing cardiac, orthopedic or pulmonary issues, we offer cutting-edge post acute care and rehabilitation services with therapies delivered daily. Our multidisciplinary team of professionals delivers the highest level care in close collaboration with leading area physicians. To enhance the comfort of our short-term patients and foster their wellbeing, our center features a dedicated Subacute Rehab Wing with many private suites, and a wide array of amenities including large flat screen TVs, delicious fine dining, and scenic grounds.
