Transitional Services includes our 20-bed Skilled Nursing Facility (SNF), which serves patients who are transitioning from the acute care hospital to another level of care. Another important component of our Transitional Services program is ContinueCare Hospital at Hendrick Medical Center, a separate, fully licensed acute care "hospital within a hospital" that provides intensive and complex medical treatment for patients. Our SNF is located on the sixth floor of Hendrick Medical Center. All rooms are private, and a family room is available to allow more space to visit with your loved one. We have a therapy gym to assist our patients in becoming stronger to return to home or the community. Our SNF is unique in that we are located within a hospital which affords all the modern conveniences and technology to better care for our patients.
