The goal of transitional care planning is to ensure disease symptoms are effectively managed and the appropriate resources are utilized through transitions between levels of care, such as from McKinley Health Center back to home. Our staff will serve as your advocate, helping you navigate through the care system, ensuring nothing falls through the cracks and that all members of the care team are in the loop and communicating effectively.
