Our Life Plan Agreement guarantees access to our senior living options such as our senior assisted living, memory support, and skilled nursing care — should the need arise. Choosing a life plan community means you will be able to stay at your community, keep your friends, and remain close to your husband, wife, or partner, if your health unexpectedly changes. In addition, should you – through no fault of your own – outlive your financial assets, your future care remains secure.

Paying a refundable up-front entrance fee provides the assurance of continued residency. This includes access to increased care, healthcare discounts, and other benefits.

Our focus at Clermont Park is to partner with residents and family members to create uniquely designed care plans that honor the priorities, preferences, and goals of each resident. We feel privileged to be a part of residents’ personal journeys.

We understand that residents’ relationships with their loved ones are essential to their health and well-being, and help to foster a community of support and care among residents, their families, and team members. Family members are welcome and encouraged to participate in regular care meetings with their loved one and their care partners, and attend community events and activities.

Our care team is trained in fall prevention measures and encourages specific daily physical movements shown to improve strength, balance, and flexibility, which help to decrease the risk for falls.
