Kern River Transitional Care

We offer 114 beds as well as a 26 bed Sub-Acute. It is part of our mission to provide transparency and understanding of a person’s journey through post-acute care. We are devoted to creating a lasting, positive impact through compassion and excellent service from our Team Members. The health and recovery of our patients at our skilled nursing facility is the primary concern of our team so it is our goal to provide each patient with compassionate, high-quality care. To be able to reach this goal, our skilled nursing professionals take a targeted and personalized approach to care by sitting down with each of our patients and their families to discuss their health care needs and treatment plans. At Kern River Transitional Care, we treat a full range of both chronic conditional and acute illnesses. With our transitional care and skilled nursing we offer our patients a wide variety of preventive programs and services to support the physical, mental, and spiritual well being of their health. Our compassionate team of physicians, nurses, therapists, social services, and support staff work hard to ensure that our patients feel comfortable in their surroundings and well taken care of.
