Azria Health Montclair is a Skilled Nursing Facility comprised of 175 beds in the Millard/Omaha area. We are a premier provider of rehabilitation and long-term care offering a breadth of services and programs. With a focus on innovation, our outstanding staff will make sure your loved one receives the very best in quality care, all while enjoying exceptional accommodations in a home-like environment. Our facility offers specialized care programs, comprehensive rehabilitation, a 14 bed Memory Care Unit, and wellness programs.
Our rehab department is comprised of physical, occupational, and speech therapy with an array of gym equipment as well as a bedroom-like set up specific to home environment to assist in transitioning our patients back to home. For additional fall protection and prevention we have the Solo Step which is a new technology that allows us to focus on dynamic rehab services to regain strength and balance in a standing or seated position, which reduces falls. The Solo Step can be used individually or with multiple patients which allows for peer to peer interactions with our patients.
