Welcome to Trinity Trace Community Care Center. So, what is culture change? Culture change is the profession’s term for person-centered care, which honors residents’ choices from dining, to bathing to natural awakenings. Our building is designed to support this philosophy in a form follows function sort of way. The building comprises a number of smaller more intimate households. Each household is surrounded by patient rooms – all private with the exception of one couples' suite–each with their own bathroom. The households have a kitchen, dining room and living area. Rather than having the institutional nursing station at the center of the households, the resident areas are at the center of the households and the caregiver spaces are to the side. So, the building is not the only change for us. Our staff has been involved in ongoing culture change trainings since the fall of last year. As our journey continues, we seek ways to provide care in a different model and we are excited about the direction in which we are moving. We invite you to visit and learn more about our journey.
