Ann Pearl's Rehabilitation Team provides evaluation and treatment of both neurological and orthopedic disorders with a comprehensive, multi-disciplinary approach. We offer inpatient physical, occupational and speech therapy. Services include discharge planning, family/caregiver training and assessments to assure a safe transition to home. Our Restorative Nursing Program helps to maintain residents at their highest functioning level. Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social workers, rehab therapists, activities coordinator and dietician. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Stimulating and meaningful social and recreational activities are offered daily to residents and adult day health participants alike. Other specialized services include advanced would care, diabetes support, IV therapy, enteral nutrition and pain management.
